Acquiring a Death, Birth or Marriage Certificate
1. Before coming to the office, please ensure that you have a ten dollar ($10) stamp and a Valid Photo ID.
2. Once you are at the office, you would be given a form to complete. (To narrow the search process,
have as much information as to name of the individual, the date and place of the event.
3. Present the completed form along with the ten dollar ($10) stamp and ID to the clerk.
4. The clerk will then search for the information and issue a certified copy of the document.
*NB: If you are taking out a certificate on behalf of someone who is on island, you would need to present a letter of consent from that person along with the persons ID as well as yours. If the person is overseas you will have to present a notarized letter of consent with proof of identification from that person as well as your ID. To avoid any inconvenience please be guided accordingly.
